Physical Education Forms

All students are required to complete this form at the beginning of the school year.

Athletic Forms

Physical Form

Any student participating in a school athletic activity must have this form on file in the office before beginning the activity

Athletic Training Rules

At BFMS, we have rules and expectations required of all our student athletes. This form must be signed and returned to your child's coach at the start of the season.

Emergency Medical Treatment Form

This form must be completed and submitted to the coach before the first athletic contest in which a player participates.

Other Forms

Non-RX Medication Authorization

This form should be completed by a parent or guardian who wishes
to allow their student to take over-the-counter medications if needed
 at school.

Rx Medication Authorization

This form should be completed by both the student's physican
and parent or guardian to allow the student to take prescription
medications if needed at school.

Emergency Medical Treatment Form

This form must be completed and submitted to the
 sponsor before the first academic competition
or extra curricular activity in which a student participates.

8th Success Form

This is the form used by 8th grade for the SUCCESS program.
