Physical Exam Forms Students involved in athletics Submitted before a student amy participate in any sports team. (Any physical exam completed after May 1st, covers the following school year.)
Insect Sting Allergy Form Students with insect sting allergy For students allergic to insect stings. Complete and submit to school nurse as soon as possible.
Medication Authorization Students needing to take medication at school For students that need to take over-the-counter or prescription medications at school.
