Wellness bloodwork should not be drawn more than one time a year unless your physician requests labs.
Additional tests available below. Please call Healthchek in advance if additional labs not listed above are requested or if you have additional blood draw questions (219) 322-7041. Although prescriptions are NOT required to participate in the wellness screenings, it is recommended that you discuss these options with your physician and complete these screenings with a physical examination.

If you need to make changes to posted schedule, first try to change times with a fellow co-worker.

No co-pay if you have Valpo School Insurance,* $ 20.00 employee (no school health insurance plan), or
$40.00 spouse/retiree.
